CJIMA treats the adult population, 18 years and older. This includes the diagnosis, treatment, and management of acute (short lasting) and chronic (long lasting) diseases such as high blood pressure, high cholesterol, heart disease, diabetes, kidney disease, anxiety, depression, and much more. We do this by seeing these patients for regular office visits and follow ups throughout the year.
Office Visits and Follow Ups is a great time to discuss specific conditions, review labs/studies, and get prescription refills. Periodic follow ups help our team stay on track with your care.
Focuses on the promotion of well-being and the prevention of disease, disability, and death. It is a pro-active effort to maintain wellness, identify risks and potential diseases so they can be caught and treated early. This is also a good time to discuss any concerns that may lead to future issues and to stay up to date with your provider and plan of care. We do this by seeing our patients on an annual basis for their physical exams and annual wellness visits. While both are preventative care visits, they do differ.
Physical Exams are intended for NON-MEDICARE patients. This is an annual proactive “well-check”. Here’s what to expect during your physical:
Annual Wellness Visit (AWV): The AWV is intended for MEDICARE patients only. Once you are insured by Medicare, this will replace your Annual Physical going forward. Wellness Visits are different from annual physical exams in that they are mainly a conversation around your overall health and priorities.
Same day urgent care is available for our established patients only. Patients seeking urgent care visits should call one of our offices first to get on the schedule or use our convenient online booking tool! Our board-certified Nurse Practitioners will often see you for urgent care. Our NPs are excellent and work hand in hand with our doctors. Urgent care visits are appropriate for non-life-threatening situations. True medical emergencies should always call 911. Examples of appropriate urgent care visits include:
While our providers do not round in the hospitals, we work with a team of dedicated physicians to see our patients when they are admitted. This ensures we have regular communication and are looped into what is happening with your care. Upon discharge, our team of nurses will reach out to you within a few days to start your discharge planning and schedule a Hospital Follow Up Visit with the practice.
It is critical to see your primary care physician after these events. A lot of things happen during hospital stays (medication changes, treatment plans and more). It is critical we are up to date and on the same page to avoid future hospitalizations and poor outcomes!
We also do pre surgical clearance visits for upcoming procedures.
CJIMA believes in taking a comprehensive approach to your overall care. Nutrition is a critical component to that.
Our Registered Dietitian (RD)/ Certified Diabetes Educator works with all types of patients: whether they are looking to maintain a healthy diet, lose weight, or develop a food plan that will assist in better managing existing conditions. Diabetes, Cardiovascular and Kidney conditions are common diseases that are perfect for an RD consultation. There is NO referral needed to see our RD. Laboratory services are conveniently available on site! Our practice has Quest Diagnostics Certified Phlebotomists at both locations. Providers use labs to diagnose, monitor and analyze your health. Patients with prescriptions from their providers are encouraged to walk in at their convenience for lab work during the following hours:
Somerset: Tuesday, Wednesday, and Friday 9am-12pm and 2pm-4pm
Monroe: Monday-Friday from 8:30am-4:00pm
Please do not bring in prescriptions from outside providers.
