Information for New Patients – What to Bring to Your First Visit
On your first visit to Central Jersey Internal Medicine Associates, please remember to bring your current insurance card, any referrals, a list of your current medications and patient records, if you have them. Please arrive 15 minutes prior to your appointment time for processing of paperwork.

It would be more convenient for you to print and fill out the following forms while at your home and then bring them with you on your first visit.

Information for Existing Patients
It is the responsibility of the patient to notify our office of any insurance and/or demographic changes.  If you have any changes to your contact information (phone number, address, etc) or your insurance information, please fill out the below form and bring with you to your visit.

Central Jersey Internal Medicine Associates can meet your healthcare needs from preventative medicine to urgent care.  In preparation for your appointment, please review the following policies.

1.  Prescription Refill and Request Policy
A  prescription refill will only be provided to patients who have had an office visit within the previous six months.  The clinical staff and your physician will review the request for a refill within 48-72 hours.  Most prescriptions are sent electronically.  Please contact the clinic at least three days before you need your refill as we need to review labs and appointment status.

We follow strict protocol for all controlled medications.  Patients must be seen by a physician every 90 days when taking a controlled medication.  Please note, patients must pick up all written prescriptions.  Patients should call their pharmacy before picking up a prescription.
Our office receives a large volume of medication requests.  We make every effort to refill prescriptions in a timely fashion.

2.  Referrals
Our office requires 3-5 business days to process referrals or authorizations.  Please call our referral line at 732-828-0002 and select option 2.  Please provide: Patient’s Name, Date of Birth, Call Back Number, Specialist’s Name, Appointment Date and Reason for Visit.

3.  No Show and Cancellation Fee
This policy has been created to help us serve you better.  A 24 hour cancellation notice is required for all appointments.  A fee of $50 will be assessed if less than 24 hours notice is given.

4.  Co-Payments
It is our policy that all co-payments be paid at the time of medical service.  Co-payment varies based on your type of insurance coverage.

5.  Medical Records
Our office requires written authorization from the patient/parent or guardian to release medical records.  Records prior to 2009 charts are stored at an offsite, secured location and may require more time to retrieve.

Med Request Solutions handles all medical record requests for Central Jersey Internal Medicine Associates and are scheduled to come to the office every other week to copy records.

How does the Patient Portal work?
The patient portal allows you to view your bills, request prescriptions and set appointments with our office. Our office will assign you a user name and a temporary password. If you do not have a user name and password, please call our office and a staff member will provide one.
