To enable us to continue with the highest quality of care we need your assistance and cooperation with the following policies.

  1. All outstanding balances have to be paid prior to your next appointment. If you have any billing issues please call (732) 828-0550.
  2. 5 day prior notification is required for all referrals and prior authorizations. This office will not fax or mail these referrals to you or any Health Care Provider/Institution.
  3.  It is your responsibility to know the details OF YOUR health plan (co-pays, referrals, etc.)
  4. Requests for prescription refills will not be taken on weekends or holidays.
  5. We require 24 hour notice for cancellation of appointments. Failure to contact the office will result in a $25 fee.  Insurance companies do not reimburse you for this fee.

NO EXCEPTIONS WILL BE MADE
