Patient-Centered Medical Home

Central Jersey Internal Medicine is one of only 500 practices in New Jersey to participate in the Comprehensive Primary Care Initiative (CPCI), and had also embarked on the journey to becoming a Patient-Centered Medical Home (PCMH).  The incentive of these programs is to lead a team-based approach, led by your doctor to increase the quality, access and coordination of care.

Our goal is to redefine the function of primary care, making it the epicenter of all your healthcare needs.  We have created a team-based approach to optimize the quality of the care you receive, personalized to your specific needs.  As a Patient-Centered Medical Home, it is our mission to manage high quality care across all spectrums of care, from preventative and well check visits to hospitalizations and specialist consultations.

By improving communication between healthcare providers within the medical community as well as patient-provider communication, we will create a care plan that is right for you.

Medicare (CPCI), Horizon, United and Amerihealth participate in PCMH. Our center will implement PCMH at our facility over a four-year period.
